Hospice & Community Care

Hospice & Community Care's mission to provide hospice services and give hope, comfort and compassion to our community when needed most. Our interdisciplinary team includes board-certified physicians, hospice-trained registered nurses and certified nursing assistants, licensed social workers, bereavement counselors and spiritual care leaders. The team focuses on quality at end of life by addressing physical, emotional, social and spiritual needs of patients and their loved ones. We have a 16-bed inpatient hospice house and also serve patients where they live; whether in their own homes, a nursing home, assisted living or other facility.
