Reliant Care Ltd is committed to providing the highest possible standards of support. Our residents will be treated as individuals and services will be delivered with respect and dignity within a safe, comfortable and homely environment that provides stimulation and encourages independence where appropriate. Reliant Care provides support that is based around each resident’s individual needs. Many of our residents are on an enhanced level of Care Programme Approach (CPA) and will have regular visits from care co-ordinators, attend local outpatient appointments and liaise regularly with the mental health services to ensure they are receiving the best quality on-going support. Reliant Care provides support that is based around each resident’s individual needs. We acknowledge that the same diagnosis may have a different impact on each individual client and adapt their support plan accordingly. We are responsive to the cultural, gender and any specific needs of the resident in developing their support plan. Whatever the need, support is provided with patience and understanding, whilst at the same time, provided within appropriate boundaries. Each resident’s support plan is exclusive, with the aim being to promote good health, increase independence and promote good quality of life. We actively encourage residents and appropriate significant family members, and support network to participate in the support planning process. Each resident is registered with the local primary health care service. If they wish to remain with their current GP and the GP is willing, residents may choose this option. Resident’s health needs, particularly medication, are kept under close and regular review. In recognising the importance of personal, individual and resident focused care, every effort is taken to ensure good communication between all members of the care team, be they nurses, carers, relatives or doctors.
