Our goal is to provide short term interdisciplinary rehabilitation for patients with a primary pulmonary or cardiac diagnosis and with acute conditions affecting their functional abilities. Patients are taught self-management strategies enabling them to reach their highest functional independence and prevent re-hospitalizations, often exceeding their prior level of function in a safe and effective manner. In addition to our comprehensive rehab, our cardio pulmonary program includes: Medical Director and Nurse Practitioner visits 4-5 times per week.
